Introduction
Cataract surgery is a very common and highly successful procedure performed to remove a cloudy lens from the eye and replace it with an artificial one. A cataract causes the eye’s natural lens, which is normally clear, to become cloudy, leading to blurry, hazy, or less colorful vision. This condition is most often a result of aging, though it can also be caused by trauma, certain medications, or underlying medical conditions like diabetes. If left untreated, cataracts can eventually lead to blindness. Cataract surgery is recommended when the cloudy vision begins to interfere with daily activities such as reading, driving, or recognizing faces. The procedure restores clear vision and significantly improves the patient’s quality of life.

Surgical Procedure
The surgery is typically performed on an outpatient basis and takes only 15 to 30 minutes. The most common technique is called phacoemulsification.
First, the eye is numbed with local anesthetic eye drops or an injection around the eye, so you will not feel pain. You will remain awake during the procedure. The surgeon makes a tiny incision (about 2 to 3 millimeters) in the cornea, the clear front part of the eye. A microscopic ultrasound probe is inserted through this incision. The probe emits ultrasound waves that break up (emulsify) the cloudy lens into tiny fragments. These fragments are then gently suctioned out of the eye.
Once the natural lens is removed, a clear artificial lens, called an intraocular lens (IOL), is inserted through the same small incision and positioned permanently in the exact same place as your natural lens. The incision is usually so small that it seals itself without the need for stitches.

Surgical Risks
Cataract surgery is one of the safest and most frequently performed surgeries worldwide. However, as with any surgical procedure, there are some risks involved, though they are generally very low.

General Complications
Common, mild, and often temporary complications include:
– Mild eye discomfort, itching, or a gritty feeling (like sand in the eye) for a few days.
– Redness or bloodshot eyes due to broken blood vessels on the surface of the eye.
– Temporary blurry vision as the eye heals and adjusts to the new lens.
– Increased sensitivity to light (photophobia) for a short period.

Rare but Serious Complications
While rare, severe complications can occur and may require further treatment or surgery:
– Infection inside the eye (endophthalmitis), which is a medical emergency that can lead to vision loss.
– Swelling of the retina (cystoid macular edema) or the cornea.
– Retinal detachment: The retina pulls away from the back of the eye, requiring immediate surgery to prevent permanent vision loss.
– Glaucoma (increased pressure inside the eye) or bleeding inside the eye.
– Posterior capsule opacity (PCO): Also known as a “secondary cataract,” this occurs when the membrane holding the new lens becomes cloudy months or years later. It is easily treated with a quick, painless laser procedure (YAG laser capsulotomy).

Pre-operative Preparation
– Medical Evaluation: Your eye doctor will perform a comprehensive eye exam and measure the exact size and shape of your eye to determine the correct focusing power for your new IOL.
– Medication Adjustments: You may be asked to temporarily stop taking certain medications, particularly alpha-blockers (often used for prostate issues), as they can affect the pupil during surgery.
– Eye Drops: You will likely be prescribed antibiotic and anti-inflammatory eye drops to start using a day or two before the surgery to prevent infection and reduce swelling.
– Fasting: You may be instructed not to eat or drink anything for 12 hours prior to the procedure.

Post-operative Care
After the surgery, you will rest in a recovery area for a short time before going home. You must have someone drive you home, as your vision will be blurry and you may be wearing a protective eye shield.

Wound Care
– Wear the provided eye shield while sleeping or napping for the first few days to a week to prevent you from accidentally rubbing or pressing on your eye.
– Do not rub or press your eye under any circumstances.
– Use your prescribed eye drops exactly as directed by your doctor. These are crucial for preventing infection and controlling inflammation.
– Avoid getting water, soap, or shampoo directly in your eye while showering.

Diet
You can usually resume your normal diet immediately after the surgery.

Post-discharge Notes
Most patients notice a significant improvement in their vision within a few days, although complete healing can take up to a month.
– Avoid strenuous activities, heavy lifting, and bending over (which increases eye pressure) for the first week or two.
– Wear sunglasses outdoors to protect your healing eye from bright sunlight and dust.
– Avoid swimming or using hot tubs for at least two weeks to reduce the risk of infection.
– Contact your doctor immediately if you experience worsening vision, severe pain not relieved by over-the-counter medication, increased redness, or sudden flashes of light or new floaters in your vision.
