截肢手術 (Amputation)

Amputation is a major surgical procedure involving the removal of a limb or part of a limb, such as an arm, leg, foot, hand, or digit. It is typically considered a life-saving measure or a last resort when a body part is irreparably damaged, diseased, or posing a severe threat to the patient's overall health. The most common cause for lower extremity amputation is severe peripheral arterial disease (poor circulation), often complicated by diabetes, which leads to tissue death (gangrene) and chronic infection. Other reasons for amputation include severe trauma (such as a crush injury or blast), cancerous tumors in the bone or muscle, or life-threatening systemic infections (sepsis). The goal of the surgery is to remove the diseased or damaged tissue, relieve pain, and create a functional "stump" (residual limb) that can bear weight and potentially be fitted with an artificial limb (prosthesis) to restore mobility and independence.

A rotator cuff repair is a surgical procedure performed to fix a torn rotator cuff, a group of four muscles and their tendons that stabilize the shoulder joint and allow you to lift and rotate your arm. Rotator cuff tears are a common cause of shoulder pain and disability, particularly in older adults due to gradual wear and tear (degenerative tears), or in younger individuals following a sudden injury or repetitive overhead sports (acute tears). Symptoms often include a dull ache deep in the shoulder, arm weakness, and pain that worsens at night or when lifting the arm. When rest, physical therapy, and steroid injections fail to relieve the pain or restore function, or if the tear is large and recent, surgery is usually recommended. The goal of the surgery is to reattach the torn tendon back to the head of the humerus (upper arm bone), thereby relieving pain and restoring shoulder strength and mobility.

Open Reduction and Internal Fixation (ORIF) is a common surgical procedure used to stabilize and heal severe bone fractures. "Open Reduction" means the surgeon makes an incision to access the fractured bone and manually realign the bone fragments into their proper anatomical position. "Internal Fixation" refers to the use of specialized hardware—such as metal plates, screws, pins, or intramedullary rods (rods inserted into the hollow center of the bone)—to hold the bone fragments securely together while they heal. ORIF is typically necessary for fractures that are complex, severely displaced (out of alignment), involve a joint, or cannot be effectively treated with a cast or splint alone. It is frequently used for fractures of the ankle, wrist, hip, femur, tibia, and arm. By providing rigid stability, ORIF allows for earlier mobility and a better functional outcome compared to non-surgical treatments for complex breaks.

Spinal fusion is a major orthopedic and neurosurgical procedure designed to permanently connect two or more vertebrae in the spine, eliminating motion between them. This surgery is performed to improve spinal stability, correct deformities, and reduce severe pain caused by movement of the unstable vertebrae. It is commonly used to treat conditions such as severe degenerative disc disease, spondylolisthesis (where one vertebra slips over another), spinal fractures, scoliosis (curvature of the spine), or following a laminectomy if the spine becomes unstable. By fusing the bones together, they heal into a single, solid bone, much like a broken bone heals. While fusion limits flexibility in that specific segment of the spine, it often significantly reduces pain and allows patients to return to a more active lifestyle.

A laminectomy, also known as decompression surgery, is a surgical procedure performed on the spine to relieve pressure on the spinal cord or spinal nerves. The surgery involves the removal of the lamina—the back part of the vertebra that covers the spinal canal—along with any bone spurs or thickened ligaments that may be causing compression. This pressure is most commonly caused by spinal stenosis, a condition where the spinal canal narrows due to aging, arthritis, or a herniated disc. When nerves are compressed, patients often experience severe pain, numbness, tingling, or weakness in the back, buttocks, and legs (or arms, if the surgery is in the neck). A laminectomy is typically recommended when conservative treatments such as physical therapy, medications, and epidural steroid injections fail to provide relief, and the symptoms significantly interfere with daily life or cause progressive nerve damage.

Carpal tunnel release is a surgical procedure performed to treat carpal tunnel syndrome, a condition caused by the compression of the median nerve as it passes through a narrow passageway in the wrist called the carpal tunnel. This compression leads to symptoms such as pain, numbness, tingling, and weakness in the hand and fingers (particularly the thumb, index, and middle fingers). When conservative treatments like wrist splints, corticosteroid injections, and ergonomic adjustments fail to relieve symptoms, or if there is evidence of nerve damage or severe muscle weakness, surgery is often recommended. The goal of the surgery is to relieve pressure on the median nerve by cutting the transverse carpal ligament, which forms the roof of the carpal tunnel. This creates more space for the nerve and tendons, alleviating symptoms and preventing permanent nerve damage.

Anterior Cruciate Ligament (ACL) reconstruction is a surgical procedure to replace a torn ACL, one of the major ligaments in the knee that provides stability. ACL tears are common sports injuries, frequently occurring during activities that involve sudden stops, jumping, or changes in direction, such as soccer, basketball, football, and skiing. When the ACL is torn, the knee can become unstable, giving way during physical activity. While some individuals with low activity levels can manage an ACL tear with physical therapy and bracing, active individuals and athletes usually require surgery to restore knee stability, return to sports, and prevent further damage to the knee's cartilage (meniscus). The surgery involves replacing the torn ligament with a tissue graft, as the ACL cannot simply be stitched back together.

Arthroscopy is a minimally invasive surgical procedure used by orthopedic surgeons to visualize, diagnose, and treat problems inside a joint. The term comes from two Greek words, "arthro" (joint) and "skopein" (to look). During an arthroscopy, the surgeon inserts a narrow tube attached to a fiber-optic video camera (an arthroscope) through a small incision—about the size of a buttonhole. The view inside the joint is transmitted to a high-definition video monitor, allowing the surgeon to see the joint's interior in great detail. Arthroscopy is most commonly performed on the knee, shoulder, elbow, ankle, hip, and wrist. It is frequently used to treat torn cartilage (like the meniscus in the knee), reconstruct torn ligaments, remove loose bone or cartilage fragments, or repair a damaged joint lining. Because it requires smaller incisions than open surgery, patients generally experience less pain, minimal scarring, and a faster recovery.

Total hip replacement, or total hip arthroplasty, is a highly successful surgical procedure designed to replace a damaged or diseased hip joint with an artificial implant (prosthesis). The hip is a ball-and-socket joint, and conditions such as severe osteoarthritis, rheumatoid arthritis, osteonecrosis (loss of blood supply to the bone), or a severe hip fracture can wear down the cartilage and bone, causing intense pain and restricted mobility. When daily activities like walking, putting on shoes, or sleeping become overwhelmingly difficult despite conservative treatments, a hip replacement is often recommended. The procedure aims to relieve pain, restore normal joint function, and significantly enhance the patient's quality of life. Modern artificial hip joints are highly durable and can last for decades.

全膝關節置換術 (Total Knee Replacement)

Total knee replacement, also known as total knee arthroplasty, is a major surgical procedure in which a diseased or damaged knee joint is replaced with an artificial joint (prosthesis). This surgery is primarily recommended for patients suffering from severe osteoarthritis, rheumatoid arthritis, or post-traumatic arthritis that causes debilitating pain, stiffness, and loss of mobility. When conservative treatments such as medications, physical therapy, and joint injections no longer provide relief, a knee replacement can significantly reduce pain, restore function, and improve the patient's overall quality of life. The artificial joint is designed to mimic the natural movement of the knee and can last for 15 to 20 years or more with proper care.
