Introduction
Hernia repair is a surgical procedure performed to correct a hernia, a condition that occurs when an internal organ, intestine, or fatty tissue bulges through a weak spot or tear in the surrounding muscle or connective tissue (fascia). Hernias most commonly occur in the abdominal wall, particularly in the groin (inguinal or femoral hernia), around the navel (umbilical hernia), or at the site of a previous surgical scar (incisional hernia). If left untreated, a hernia can grow larger and become increasingly painful. More importantly, it carries the risk of becoming “strangulated,” a life-threatening complication where the blood supply to the bulging tissue is cut off, requiring emergency surgery. Hernia repair surgery aims to push the bulging tissue back into place and reinforce the weakened muscle wall.

Surgical Procedure
Hernia repair can be performed using two primary techniques: open surgery and minimally invasive (laparoscopic or robotic) surgery.
In open hernia repair, the surgeon makes an incision directly over the site of the hernia. The protruding tissue is gently pushed back into the abdominal cavity. The surgeon then repairs the weakened muscle wall by sewing it together. In most modern procedures, a synthetic surgical mesh is placed over the weakened area to provide additional support and significantly reduce the risk of the hernia recurring. The incision is then closed with stitches, staples, or surgical glue.
In laparoscopic hernia repair, the surgeon makes three to four small incisions near the hernia. The abdomen is inflated with carbon dioxide gas to provide working space. A laparoscope (a camera) and specialized surgical tools are inserted through the incisions. The surgeon repairs the hernia from the inside, placing and securing a mesh patch over the defect. This minimally invasive approach often results in less post-operative pain and a faster recovery.

Surgical Risks
Standard surgical risks apply to hernia repair, including adverse reactions to general or local anesthesia, bleeding, and the risk of blood clots forming in the legs (DVT).

General Complications
Common complications following a hernia repair include:
– Pain, swelling, and bruising around the surgical site or in the groin area (especially in men after inguinal hernia repair).
– Temporary difficulty urinating (urinary retention), which may require a temporary catheter.
– Infection at the incision site or deep around the surgical mesh.
– Seroma or hematoma: A buildup of fluid or blood under the skin at the surgical site.

Rare but Serious Complications
While uncommon, serious complications can arise:
– Chronic, long-term pain caused by nerve damage or entrapment by the surgical mesh or stitches.
– Recurrence of the hernia, requiring further surgery.
– Damage to surrounding organs, blood vessels, or the vas deferens (the tube carrying sperm in men) during the procedure.
– Mesh rejection or migration, though extremely rare with modern materials.

Pre-operative Preparation
– Fasting: Patients are typically required to stop eating and drinking 8 to 12 hours before the procedure.
– Medical Evaluation: Routine blood tests, an ECG, and a physical examination are conducted to ensure fitness for surgery.
– Medication Adjustments: You must inform your doctor about all medications you take. Blood thinners, aspirin, and certain anti-inflammatory drugs may need to be stopped several days prior to surgery to minimize bleeding risks.
– Smoking Cessation: Smokers are strongly advised to quit or pause smoking before surgery, as smoking impairs healing and increases the risk of hernia recurrence and respiratory complications.

Post-operative Care
Most hernia repairs are performed as outpatient procedures, allowing patients to go home the same day once they are awake, walking, and able to urinate. Pain management is crucial; doctors will prescribe pain relievers or recommend over-the-counter options. Ice packs can be applied to the surgical area to reduce swelling and discomfort.

Wound Care
– Keep the incision area clean and dry. Showers are usually permitted after 24 to 48 hours, depending on the surgeon’s instructions.
– Avoid baths, hot tubs, or swimming pools until the wound is completely closed and healed.
– Do not apply lotions, powders, or ointments directly to the incisions.
– Watch for signs of infection, such as spreading redness, increasing pain, warmth, or pus drainage.

Diet
– Resume a normal diet gradually, starting with light, easily digestible foods if you feel nauseous from the anesthesia.
– A high-fiber diet (plenty of fruits, vegetables, and whole grains) and adequate fluid intake are extremely important to prevent constipation. Straining during bowel movements puts pressure on the surgical repair and must be avoided. Stool softeners may be recommended.

Post-discharge Notes
Recovery time depends on the surgical approach; laparoscopic patients often return to desk work in 1 to 2 weeks, while open surgery patients may need 2 to 4 weeks.
– Strict lifting restrictions are vital. Avoid lifting anything over 10 to 15 pounds for at least 4 to 6 weeks.
– Avoid strenuous exercise, core workouts, and intense physical labor during the initial healing phase.
– Light walking is encouraged daily to promote healing and prevent blood clots.
– Contact your doctor if you experience a fever over 101°F (38.3°C), severe pain not relieved by medication, difficulty urinating, or significant swelling at the incision site.
