Healthy Wealth: Understanding Fever (2) 2018-6-21 | Dr. Chan Yan Wing

Cold water bath? Cooling patch? Ice pack application?

Pouring cold water, using cooling pastes, or applying ice packs are not effective. However, they can to some extent help relieve discomfort caused by fever symptoms. When children have a fever, they inevitably feel unbearable heat, so parents often use methods such as cooling pastes or ice packs, hoping to reduce their child's discomfort. However, similar methods have no curative effect on the disease behind a high fever. Parents should also be aware that "pouring cold water" will cause the subcutaneous blood vessels to constrict, reducing heat dissipation. Therefore, bathing a child with normal water temperature when they have a fever can achieve a cooling effect.

Pay attention to the patient's mental state, it's even more important.

Paediatric fever reducers can generally be divided into suppositories and oral medications, and parents can choose suitable options for their children depending on the situation. Suppositories have an oily texture, the advantage being that they are easily absorbed, work faster, and have a longer-lasting effect. If a child is very young or has symptoms of vomiting making oral medication difficult, suppositories can be chosen. When a child is fast asleep, using suppository-form fever reducers can avoid waking them up; however, parents must ensure correct suppository insertion technique to guarantee the medication is successfully placed in the anus. Oral medications are more convenient and do not require any special administration techniques, but they are absorbed more slowly and take longer to take effect.

Dr. Bosco ChanIt is important to observe a child's overall condition rather than just their temperature. When children experience a high fever, parents often become very worried and anxious, frequently wondering "Why isn't the fever going down after taking medication?", and may even suspect that the medication is not effective. In fact, depending on the cause of the fever, the duration of the fever can vary. Some fevers may repeatedly go down and then come back up, and not all illnesses will see a rapid drop in temperature after taking medication.

A fever's temperature is not an absolute indicator of how severe an illness is. In other words, a higher temperature does not automatically mean a more serious condition. For example, if child A has a higher fever but is in good spirits, eating and drinking well, and can talk and play, while child B has a lower fever but is drowsy and listless, doctors will be more concerned about child B's condition. Therefore, parents should pay more attention to their child's overall state rather than just the high fever symptoms.

Furthermore, parents should pay close attention to their child's fever trend after taking medication. If the child's fever fluctuates after medication but the temperature at each instance of fever is consistently decreasing, this indicates that the medicine is gradually taking effect but has not yet managed to bring the child's temperature down to normal levels. This does not mean the fever reducer is ineffective. At the same time, a continuously decreasing fever trend also indicates that the child's condition is improving, and parents need not worry excessively about "why is my child still feverish?"
